Long Term Services and Supports Case Manager (Full Time)

Location: Franklin County/North Quabbin Area

General Description:

The long-term services and supports case manager is responsible for functioning as a member of the integrated care team. The position provides evaluation of the need and eligibility of members for support and services in the community, while acting as an advocate as needed. The long-term services and supports case manager promotes independent living and total wellness, working with people with disabilities and elder populations. The position assists elders and individuals with disabilities to identify and attain personal goals necessary to promote good health and enhance their ability to live independently in the community. Frequent driving throughout the assigned geographical area is required.

Qualifications

The ideal candidate will have excellent interpersonal skills and be able to work well independently and as part of a collaborative team. Computer literacy, ability to multitask, and strong organizational skills required. LSW license or bachelor’s degree in relevant field required with 2 years’ experience. A valid driver’s license and reliable transportation is required.
