Congregate Housing Coordinator/Case Manager (Full Time)

Location: Franklin County & North Quabbin Area

General Description:

This is an exciting position, serving Franklin County and the North Quabbin area. The role is a 50/50 split of a 35-hour work week. The housing coordinator provides oversight of a congregate residence, providing recruitment and assessment of applicants; community outreach; community-minded planning; and facilitation of individual and group services. The case manager component of the role is responsible for needs assessment evaluation; implementation of services; care plan setup/monitoring; and functioning as the central source of information and communication.

  • Responsiveness & crisis intervention skills

Bachelor’s degree in relevant field required, as well as previous experience in Health or Human Services. License in social work or MSW preferred. Must have valid driver’s license, reliable transportation, and able to travel throughout the assigned territory.
