Care Coordinator (Full Time)

Location: Wachusett & North Quabbin Area

General Description:

The Care Coordinator works independently and as part of an interdisciplinary team providing long-term service supports, case management, and administrative support for MassHealth-funded programs. The Care Coordinator is primarily a remote-working/field position, conducting assessments in a home setting and coordinating services. The position serves consumers in Wachusett areas; Fitchburg, Gardner, Leominster, Winchendon, and North Quabbin areas; Athol, Orange, Phillipston, etc.

Must have bachelor's degree and a willingness to learn and serve elders and/or persons with disabilities. Successful candidate will have reliable transportation and ability to frequently travel throughout assigned areas.

Compensation and Benefits

This M-F 35-hour work week position has remote work and flex schedule options, with competitive pay, excellent work environment, and an outstanding benefits package, including health, dental, vision, pet insurance, company-paid life insurance, short-term disability, and HRA.
