Full Time—Hybrid/Remote Work Model

LifePath seeks a full-time case manager. Ideal candidates will be comfortable working independently and as part of an interdisciplinary team, providing case management and administrative support for the Home Care program. The position serves older adults in Franklin County and the North Quabbin area.

Demonstrated experience providing case management support to older adults. Must have a commitment to supporting people to live safely and independently in the community. Knowledge and experience in person-centered planning a plus. Bachelor’s degree in relevant field preferred. Successful candidate will have a valid driver’s license, reliable transportation, and ability to frequently travel throughout assigned areas. Use of personal cell phone, data, and downloading of company-provided applications required.
Compensation and Benefits
This is a full time position (35 hours per week); starting pay is $21.60 per hour.
