  • Full-time Adult Family Care (AFC)/Shared Living Program Director. AFC and Shared Living are core programs to consumers and integral to LifePath’s mission and values. The program director is responsible for fiscal and operational oversight, quality compliance, outreach and growth strategy and oversight of staff management.  Qualifications include a bachelor’s degree and a minimum of five years of professional health-care experience working with elders or adults with disabilities or equivalent.

  • Congregate and Home Delivered Meals Office Coordinator/Assessment Worker provides coordination, monitoring, clerical assistance, data entry and Nutrition program backup as needed, 8-10 hours per week. This person is responsible for in-home assessing and re-assessing eligibility for Non-State Home Care Program referrals for home-delivered meals, and referring participants for other services as needed. A high school diploma and valid driver's license with reliable transportation is required.

  • Per diem nurse. No nights and no weekends! LifePath provides support services to elders and people with disabilities. Currently seeking Per Diem RNs/LPNs for PCA & AFC program. This position works as part of an interdisciplinary and cross functional team. Territories include Franklin, Hampden, and Hampshire counties and the North Quabbin/Mount Wachusett area.
