CARE (Coordination, Advocacy, Referrals, and Education)

CARE can help anyone in need of assistance with:

• Assessment of care needs at home
• Designing a service plan
• Arranging for skilled services through an agency or with individuals
• Skills training to help with hiring and managing caretakers
• Ongoing home visits
• Adjustments to care plans and services as needed

A tailored package will be designed to meet individual needs.
