My Life, My Health - Chronic Disease Self-Management
"My Life, My Health" is a workshop series designed for those with chronic physical and/or emotional challenges that impact quality of life, including diabetes, arthritis, weight issues, high blood pressure, heart disease, and COPD. Subjects addressed include:
- How to manage pain and fatigue
- Nutrition and exercise options
- Improving communication with medical providers and family
- Using effective relaxation and breathing techniques
- Managing medications
- Making sound treatment decisions
Workshops are highly interactive, provide mutual support and build participants' confidence to manage their health and remain active. Participants set goals and create weekly “action plans” in a very supportive, fun atmosphere. Caregivers and family members are also welcome in the program. Workshops meet weekly for six weeks.
"My Life, My Health" was developed by the Stanford Patient Education Research Center. It is an evidence-based program that has been researched and proven to have positive results for participants.
