Care Transitions

You can get help coming home from the hospital, rehab center, or nursing facility.

If you are returning home after a stay at a care center, you can get help with managing the transition to help ensure your return home is successful and comfortable.

Programs include:

  • Transition in Care Program: Eligible members can receive four weeks of care coordination when discharging from the emergency room.

  • Community Transition Liaison Program: Residents 22 years old and older residing in a nursing facility can receive support with transitioning home.

  • Options Counseling Program: People over 18 years old can meet with a specialist who can provide community resources.
