Personal Protective Equipment Distribution Policy for Consumers
Personal Protective Equipment (PPE) is one important component of reducing community spread of COVID-19 and other viruses and illnesses. LifePath understands this need and remains committed to providing PPE to those we serve. Requesting PPE PPE can be requested once every other month in the following quantities:
● 20 Surgical Masks
● 10 N95 / KN95 Masks
● 6 COVID-19 Rapid Tests
Note: Test kits vary in number of individual tests included. You will receive 6 individual rapid tests per request, typically contained in 3 kits.
● * 50 Pairs of Gloves
○ Note: Gloves are only available if requested through LifePath program staff.
There are two methods for individuals receiving LifePath services to request PPE:
● Request PPE through your LifePath program staff. The LifePath staff member will submit the request on the individual’s behalf using internal request procedures.
● Request PPE directly using the form above.
LifePath’s PPE distribution is meant to supplement an individual’s other sources of PPE, when available. Requests for quantities larger than those stated above will be considered and filled at LifePath’s discretion. LifePath has various sources for replenishing PPE supplies. As a result, your full request may not be provided due to limited quantities on hand. Glove size requests will be honored when possible. Alternate sizes may be substituted if requested size is unavailable. LifePath reserves the right, in its sole discretion, to modify this policy at any time.
