LifePath is proud to offer EnhanceWellness Coaching, our first one-on-one wellness program that offers free, individual, long-term, and personalized coaching to support behavior change. This program encourages older adults to take on life’s challenges and manage their chronic health conditions.

The EnhanceWellness program has undergone scientific evaluation and has proven to be effective with measurable results. Benefits of the program include improved self-ratings of health, increased levels of physical activity, increased social interactions, improved activities of daily living, and a decrease in depression.

Participants develop skills in self-management and grow confidence to manage their health through:

  • One-on-one work with a trained Wellness Coach by phone, video, or email
  • Development of a personalized health action plan that identifies health risks and steps to take to improve health and well-being
  • Weekly troubleshooting by collaborative problem-solving

Over a period of six months, a participant and their Wellness Coach decide what areas of behavior the participant wants to address, and then they work to find solutions to an individual’s barriers to change. The emphasis is on personal choice and responsibility for deciding how to integrate healthy behaviors into daily life as a way to manage chronic illness.

This collaborative approach, with the Wellness Coach serving as a “guide,” supports self-management through encouragement, problem-solving, decision-making, and action planning.
