The Behavioral Health Outreach for Aging Populations program can help you with:

  • One visit or multiple visits
  • Connections to other resources that may help
  • Appointments in your home, at our office, or via virtual meeting
  • Assistance navigating the service system

This program is free of charge.

Additional supports include Dementia Coaching Sessions and Support Groups, which provide information and tips to help you care for a loved one.
